Individual Case Management
Personalized Support
Our case management services provide individualized guidance to help clients identify their needs, set achievable goals, and access essential services. Through ongoing support and follow-up, our team works closely with individuals and families to connect them with housing, healthcare, financial assistance, and other resources that promote stability and long-term well-being.
Immediate Emergency Assistance
Urgent Relief
We provide emergency assistance for individuals experiencing financial or personal crises. Support may include rental assistance such as first-month rent deposits, help with RG&E back payments, and funeral assistance of up to $2,500 for families in need. Services are available to qualified individuals through Medicaid eligibility and pre-screening, with language support available in Swahili and French.

We help individuals navigate available services and provide referrals to programs offering housing assistance, food and clothing support, healthcare access with interpretation services, and transportation assistance.
