Through one-on-one guidance, emergency assistance, and referrals to community services, our team works to ensure clients have access to housing support, healthcare, basic needs, and other critical resources that promote stability, well-being, and long-term success.
Individual Case Management

Our case management services provide individualized guidance to help clients identify their needs, set achievable goals, and access essential services.
Immediate Emergency Assistance
Urgent Relief
We provide emergency assistance for individuals experiencing financial or personal crises. Support may include rental assistance such as first-month rent deposits, help with RG&E back payments, and funeral assistance.
Referrals to Community Resources
Community Connections
Our team connects clients with trusted community resources that support housing stability, healthcare access, and basic daily needs.
Partnering with Social Care Network
Through our partnership with WeLinkCare by Forward Leading IPA (FLIPA), we help eligible Medicaid members connect with services that address everyday needs affecting their health and well-being. Our team can complete a social care screening, identify areas where support may be needed, and guide individuals through the eligibility and referral process.
Active Medicaid enrollment is required.* Services are based on screening results, individual eligibility, identified needs, preferences, and program availability.
Social Care Screening
We begin with a confidential screening to identify health-related social needs, including challenges involving food, housing, and transportation. Medicaid members are encouraged to complete a screening annually or after a major life change.
Housing Support
We help individuals identify housing-related needs and connect with appropriate resources. Depending on eligibility, support may include housing navigation, help addressing unsafe living conditions, tenancy-related services, or referrals to local housing programs.
Food & Nutritional Support
Eligible members may be connected with food and nutrition resources that support healthier living. Assistance may include referrals to community food programs, nutrition services, and enhanced food supports based on eligibility and availability.
Transportation & Navigation
Our Social Care Navigators help eligible members access transportation and other community services needed to support their health. Navigators can explain available options, coordinate referrals, and provide ongoing assistance connecting with qualified service providers.
*Social Care Network services are available at no cost to individuals with active Medicaid coverage. All Medicaid members may complete a screening, but eligibility for specific or enhanced services is determined through an additional assessment. Services are provided according to identified need, program requirements, and availability.
